Community Health/home care Nurse Term Position Job posting 18-31
We are currently seeking an energetic and highly motivated individual to join our team as the Community Health/Home Care Nurse. Under the direction of the Director of Health, the CHN/HCN is responsible for providing, participating and directing staff in the delivery of a high standard of community health services. The CHN/HCN provides assessments for community band members needing assistance at home. The services provided are client and family focused including immunization, pre-post natal care, communicable disease control in Community Health; also support for initial assessment, one-on-one teaching, treatment, and rehabilitation to prevent deterioration and promote independence in home care nursing.
Plan and implement Community Health Programs in conjunction with the community health team with the objectives, guidelines, procedures and policies as adopted by the community:
Promote self-care of clients and encourage participation in health decisions and goal setting for care;
Provide direction and supervision to the Home Community Care Staff & program;
Maintain confidentiality of all work and client related information in accordance with policy.
Bachelor of Nursing
Current license to practice and in good standing with S.R.N.A.
Computer literacy, including effective working skills of MS Word, Excel and e-mail.
Ability to work well with others in a team approach, and adapt to changing situations.
Maintain sensitivity to clients’ cultural and/or political background.
Valid Saskatchewan Driver’s license and access to a reliable vehicle.
Ability to plan, organize, and manage time effectively as well as ensuring maintenance of required records.
